Prescription Coverage and an Accidental Dealth Life Benefit.
Blue Selections Basic
The Blue Selections Basic
This plan offers $1000, $2500, $5000 and $10,000 deductibles. It pays at 70/30
for co-insurance, and has a $500 deductible for Prescriptions then 50% coverage.
Blue Selections Plus
The Blue Selections Plus
plan offers $500, $1000, $2500 and $5000 deductibles. It pays at 80/20 for co-insurance,
and has a no deductible for Prescriptions with 50% coverage.
Blue Selections Premier
The Blue Selections
Premier plan offers $500, $1000, $2500, $5000 and $7500 deductibles. The
plan has $20 office visit and urgent care copayments. As well as $20 copayments
for Annual Women's Exams, Well Baby Care and Immunizations. It pays at 80/20
for co-insurance, and has a no deductible for Prescriptions with 50% coverage.
Regence Health Savings Account
The HSA Qualified plan is the HSA eligible
plan offered by Regence BlueCross BlueShield of Oregon. The plan
offers 3 different deuctible options for individual and family plans. The
plan also offers an Accidental Death benefit.
